To better assist you, the following carriers have 24 hour direct claim reporting services available for Automobile, Workers Compensation and Business Owners Policies. Prior to calling, please have your policy number and claim details available for the claims adjuster. The insurance carrier will forward a copy of your claim report to our office.

To report all other claims please complete the form below and fax to 888-625-3501.  We will attach all the policy information and route it to the proper company for the most efficient handling.  Due to the special forms we have for our program, if claims are sent to the general reporting numbers listed below, significant delays can occur while they try to route the claims to the proper claims unit.
